Pursuant to the state Open Records Law, Wis. Stat. Ann. Sec. 19.31 to 19.39, I/We am/are requesting copies of the following records, concerning:

The Mead County Library board, Sheboygan Mayor's personal and office records, as well as all of Susan Hart's notes concerning Sharon Winkle's contract and/or agreement for employment, including billing and/or negotiations with Mary Lynne Donohue, changes to the Mead library's budget, and ramifications to services due to budget changes.

Dates for records requested:      APRIL 25th to Present

These records would include any material on which written, drawn, spoken, visual or electromagnetic information is recorded or preserved, regardless of physical form or characteristics, which has been created or is being kept by any authority. The term further includes, but is not limited to, handwritten, typed or printed pages, maps, charts, photographs, films, recordings (on private and/or public equipment), tapes (including computer tapes), computer printouts and optical disks. This list also includes calendars, phone messages, notes, letters, faxes, minutes of meetings, e-mail messages (which have been sent, received or deleted), and/or phone logs, including numbers received and dialed of any and all parties involved. These records must be obtained from both personal and public sources, both before and during any applicable position's term of office. Please provide as much information gathered onto CD-ROM whenever possible, and advise if any copy costs involved exceed $50.

As provided by the above-mentioned Open Records law, I/we will expect your response as soon as practicable, and without delay. I/we will assume that if I/we don't hear from you by

June 14, 2006 _ , which is 14 days from this request, that you will have denied this request. If you choose to deny this request, please provide a written explanation for the denial, including a reference to the specific statutory exemption(s) upon which you rely. Also, please provide all segreable portions of otherwise exempt material.

Please be advised that I/we am/are prepared to pursue whatever legal remedy necessary to obtain access to the above-mentioned records. Violation of the Open Records law could result in the award of court costs, attorney fees and actual damages of not less than $100. If a court determines that noncompliance with the law was arbitrary or capricious; it may award punitive damages and attorney fees, as well as issue a fine of up to $1,000.
